Four Inova Hospitals earn top safety grades from The Leapfrog Group
Four Inova hospitals have received an “A” Hospital Safety Grade for the fall 2025 term from The Leapfrog Group, a national nonprofit organization that evaluates hospitals on patient safety. Inova Alexandria Hospital, Inova Fair Oaks Hospital, Inova Fairfax Hospital, and Inova Loudoun Hospital each earned the top grade, which reflects performance on measures related to errors, accidents, injuries, and infections.
In addition, Inova Fair Oaks and Inova Loudoun hospitals were named among Leapfrog’s Top Teaching Hospitals for 2025, an honor recognizing facilities that combine high safety standards with educational programs for medical trainees.

Inova Loudoun Hospital is one of only 11 hospitals nationwide—and three in Virginia—to have received straight “A” grades since the program began, marking 28 consecutive grading periods. Inova Fair Oaks Hospital has earned 27 consecutive “A” grades, while Inova Fairfax Hospital has maintained the top rating for 15 terms.
“Earning an ‘A’ Grade means these Inova hospitals made a true commitment to put patient safety first,” said Leah Binder, president and CEO of The Leapfrog Group. “We congratulate the leadership, Board, clinicians, staff and volunteers that all had a role to play in this achievement.”
The Leapfrog Hospital Safety Grade stands as the only hospital ratings program focused solely on preventable medical errors, infections and injuries that kill more than 500 patients a day in the United States. This program is peer-reviewed, fully transparent and free to the public. Grades are updated twice annually, in the fall and spring.
